Emergency contacts, background check authorization, participant agreement, etc.

ID/Documents needed: Medical insurance card, SS#, Drivers License, Payment card for $300 Deposit

You may also pay the $300 deposit through this link. Select “GCA Missions Fund (Donations from Individuals)” in the “To:” dropdown and type in participant name and mission trip destination in memo area.

(Optional) Dr, Mike Stewart has prepared a letter encouraging your church to cover part or all of the mission trip expense. Please download and offer to your stewardship committee or other entity as appropriate.
